Service Description

EMR Immediate Onyen Access Removal

Use this form to send requests that require action from the Information Security team to deprovision or disable user account-related services when an employee leaves their current position and access needs to be removed urgently. 

This service does not initiate any HR-specific actions for departing employees. Please reference your internal HR process for how to initiate HR-specific actions and to determine who is responsible for submitting this request for your unit. HR access and any additional enterprise-level access, such as the ARC process, must still occur for an employee’s access to be fully removed. 

The “Requested for” user will receive email notifications about this request and should be the person who is responsible for ensuring the request is completed (not the departing employee). 

If the departing individual is a student this request will not be completed as they will retain an active affiliation as a student. 

 

Employee Access Removal

This service does not initiate any HR-specific actions for departing employees. Employee Access Removal is needed when employees or affiliates of UNC leave, change affiliation, retire, or take a position in another department. This service will send a request to service providers (typically departmental IT groups) to deprovision computer and user account-related services when an employee leaves their current position. This request can take a few business days and should not be used when the request is urgent or if there is a loss of trust with the employee.

This service requests the deactivation of all user accounts and access and the reclamation of computing devices assigned to the departing staff member.
