F&O Mobile Device Request

Service Description

New Mobile Device or Line Change Request for departments currently providing mobile devices for users.

The following information for the new user is required to complete this form:

• Onyen
  • If the new employee has not created an Onyen yet, they can do so here: https://onyen.unc.edu/
  • A valid PID is required to create an Onyen. If the employee does not have a valid PID, please check with your HR representative.  
• FOIT cannot deploy a phone without a valid Onyen. Please do not submit this form until you can provide a valid Onyen.
